Who can complete this form
The existing nominee for the minor gaming permit you want to change must complete this application.

When to use this form
Use this application form if you wish to make changes to any conditions on an existing minor gaming permit for a:
- Raffle
- Lucky envelope,or
- Fundraising event.
IMPORTANT INFORMATION
You cannot change your minor gaming activity until we formally advise you that your application has been approved and your permit has been amended.
If you are proposing to change conditions after ticket sales have started, we will look at whether the changes you want to make will be detrimental to people who have already purchased tickets. Please include clear advice of the changes you want to make.
What type of changes can you request
Changes you can apply for include:
- the type and value of prizes being offered
- the dates of ticket sales or prize draws
- the location of an event or draw.
Application cannot be saved in draft form
This application needs to be completed and submitted in the one step. It is not possible to save this application in draft form and return to it later.
Please be prepared to make a payment at the time you lodge this application (see payment methods below).
False or misleading information
It is an offence under the Act to give information that is false or misleading. If you give false or misleading information, you may be prosecuted and fined up to 60 penalty units.

Publication of disciplinary action
If disciplinary action is taken against a permit holder the VGCCC's decision,
including the name of the community or charitable organisation which holds the permit, will be published on the VGCCC’s website
and in the VGCCC’s Annual Report.
